Multiple endocrine neoplasia?
Look, I'm guessing out of left field here, but I think I remember you mentioning that Joan had both thyroid and adrenal gland problems, which makes me think of Type 2 multiple endocrine neoplasia (MEN). I'm also not sure of Tim's history, just remembering that he's been having a really hard time and a lot of nausea. There is a condition called Zollinger-Ellison syndrome that is associated with Type 1 MEN that has a lot of Crohn's-like symptoms. If anything about Tim's case of Crohn's is been atypical, and Joan does happen to have Type 2 MEN, it might be worth asking Tim's GI to make sure to rule out Zollinger-Ellison.Sorry, I'm sure Joan really doesn't need armchair (non)doctors butting in, it's just that there seems to be some uncertainty about the diagnosis, and you never know what might be helpful.
